v Our RN staff has experience in managing patients at home safely, and we can arrange weekly, bi-weekly or monthly visits to check on the status of medications, health status, draw labs, or anything that is needed to help your loved one remain at home. We can also provide home health aide services at an affordable rate, or supervise and train your own private caregiver. Of course, with the patient’s permission, this can all be communicated to you on a regular basis.
v Implementation of in-home monitoring devices.
o Lifeline Personal Emergency Response System enables the senior to remain in their own home longer—where we would all want to be. The Lifeline ensures that prompt, caring help is available at the press of a button, 24 hours a day, 365 days a year. Also, Lifeline with reminders helps manage medications and provides the peace of mind needed to remain active and vital as long as possible. o Telehealth Monitoring program will monitor vital signs (blood pressure, heart rate, oxygen saturation, weight) on a daily basis and will detect significant changes earlier to prevent hospitalizations. This data can also be communicated to the primary health care provider.
